Please fill out this form as complete as possible. The information will be used in our two required forms…Medical Release Form and Registration Form. In the event of an emergency we will always attempt to contact the parents first, however if parents are not available we would like to have two other Emergency Contacts. Once complete, save the document with your child’s Last and First name and then email to us at: aztecdiverssandiego@gmail.com
One we input the information into our database, we will then merge the information into our two documents and attach to an email ready for your signature and initial. You may then mail all Forms to our office with a copy of AAU card and check for fees. Note, AAU registration is required and costs $16/yea
